Betacor

Composition: Each tablet contains: Sotalol HCL 80mg.

Properties: Sotalol hydrochloride has class II and III anti-arrhythmic properties and non-selective adrenergic beta receptor blocking action.

Pharmacodynamics:
Sotalol hydrochloride (Betacor®) has two main actions;
-    Beta-blocking action with class II anti-arrhythmic property:  It increases sinus cycle length (slows heart rate), decreases A V nodal conduction and increases A V nodal refractoriness.
-    Class III anti-arrhythmic action:  It prolongs atrial and ventricular action potentials and prolongs the atrial and ventricular refractory periods through blocking potassium channels (phase 3).

Pharmacokinetics:
Sotalol Hydrochloride is completely absorbed after oral administration and has an absolute bioavailability of 100%. Peak plasma levels occur within 2-3 hours after oral intake.  Plasma protein binding is about 35%. Sotalol Hydrochloride is excreted mostly unchanged in urine and the elimination half-life ranges from 7-18 hours which is increased in the elderly and in renal failure.  Sotalol Hydrochloride can cross the placenta and is excreted in breast milk.

Indications: -    Sustained ventricular tachycardia
-    Ventricular tachy-arrhythmias
-    Paroxysmal supra-ventricular tachycardia
-    Wolff-Parkinson White syndrome (WPW syndrome)
-    Recurrence of atrial fibrillation
-    Angina pectoris and essential hypertension

Contra-Indictions: -    Atrio-ventricular block
-    Heart failure
-    Bronchial asthma
-    Diabetic ketoacidosis and metabolic acidosis
-    Pregnancy and lactation
-    Hypersensitivity to the drug

Adverse Effects:May be present due to its ?-blocking action e.g. hypotension, bradycardia and conduction disturbances in  A-V node.

Dosage & Administration: The recommended oral dose of Sotalol Hydrochloride is 160mg once daily or 80mg twice daily.  This dose may be increased up to 480mg/day for emergency treatment of cardiac arrhythmias.
